Procedures of alien registration
Where to register
Citizen Section of Main Office or Citizen Life Section of General Branch.
You have to apply for alien registration in the following cases:
When you come to Japan
Register within 90 days when you entered Japan .
You have to bring:
·Passport ·2 photos (4.5cm×3.5cm) (Only passport is needed for those who are under the age 16)
When you have a new baby born in Japan
Register within 60 days when your baby is born.
You have to bring: birth certificate (attached in the birth report)
When you move to Osaki City from other city
Register within 14 days.
You have to bring: alien registration certificate
When you move inside Osaki City
Register within 14 days.
You have to bring: alien registration certificate
When you move from Osaki City to other city
You do not have to visit the City Hall in Osaki City .
Go to the government office where you move to with your alien registration certificate to complete the procedure.
When you change your name or nationality
Register within 14 days when your name or nationality is changed.
You have to bring:
·alien registration certificate ·Passport or other documents to prove your name and nationality is changed.
·2 photos (4.5cm×3.5cm)
When the visa status is changed or extended
Register within 14 days if case your visa status is changed or extended.
You have to bring: ·alien registration certificate ·passport
When your job, name or address of your company is changed
Register when your change your job or company.
(Except for permanent resident or special permanent resident.)
You have to bring: alien registration certificate.
When your alien registration card is expired
Register within 30 days from the day you become 16-year-old or from the designated day.
In addition, you have to register if your visa is changed after your become 16-year-old.
You have to bring: ·2 photos (4.5cm×3.5cm)·passport
When your alien registration card is stained
A new alien registration certificate will be issued.
You have to bring: ·2 photos (4.5cm×3.5cm)·passport
When you lose your alien registration card
First, report to a police station and request a file number.
In case your alien registration certificate is not found, apply for a new one.
You have to bring: ·2 photos (4.5cm×3.5cm)·passport
When a foreigner is dead
After you report someone's death in city hall, return the alien registration certificate of the person.
You have to bring: alien registration certificate.
When you need a copy of alien registration certificate
You have to bring: identity certificate of the applicant
Qualified applicants: the said person, family member living together or an agent with an authorization letter signed by the said person. For more information

Apply your national health insurance within 14 days
The window for application is at Citizen Section, or the Citizen Life Section in General Branches.
You have to bring the followings to complete the procedure.
When you join the national health insurance
（1）When you move in, bring the certificate of change of address from where your were.
（2）When you quit the health insurance program from your company, bring the certificate of resignation or certificate to show you quit the social insurance program.
（3）When you have a new baby, bring the mother and child handbook and bank account book (except post office) of the householder.
（4）When your social relief is stopped, bring the official letter of social relief.
If there are other members in one single National health insurance account, bring the national health insurance card.
When you cancel your National health insurance
（1）When you move out, bring your national health insurance card
（2）When you join the health insurance program of your company, bring your National health insurance and the health insurance card from your company.
（3）When someone is dead, being the national health insurance card, death certificate of death, and the account of the person who runs the funeral (except post office account).
（4）When you begin to receive social relief, bring the national health insurance card and the official letter of social relief.
When you lost your National health insurance card
Bring a document to show your identity.
E.g. Driving license
Bring the National health insurance card in following cases:
（1）When you change your address in Osaki City.
（2）When your name is changed.
（3）When the householder is changed.
（4）When the household is combined or separated.
（5）When you move to other place for study, or in case you need a separated national health insurance card. （In this case, you need to bring your student registration certificate.）
（6）When you need a separated national health insurance card due to working in other place.
National health insurance card is the important document to show that you are protected under the National health insurance program. For more information
